Therefore, we request that all payment arrangements be made for our office services at the time they are rendered unless prior agreement has been made. We accept most forms of insurance as well as major credit cards for your convience. If you have questions about payment please contact our office or your insurance provider for further details.
Please request all prescriptions and authorizations during our regular office hours. Renewals requested at other times will be filled only for extreme or emergent circumstances.
